This role as a qualifies as a Center of Excellence (CoE) position, which provides clinicians with a structured pathway to develop expertise in a specific program, niche, or service line while maintaining a reduced caseload. The CoE model is designed to foster both professional growth and organizational innovation by allowing clinicians to dedicate time to program development, leadership, and specialized service expansion. We are seeking a Transfer Specialist—a dual-role clinician who will maintain a personal caseload while facilitating timely and appropriate therapist transfers for clients requesting a change
The Transfer Specialist will play a key role in ensuring smooth transitions for clients seeking a new therapist, as well as supporting clinicians who are leaving the agency by coordinating transfers for their caseloads. This individual will collaborate with outgoing clinicians, supervisors, and directors to assess client needs and match them with an appropriate provider. Additionally, they will assist in developing policies and best practices to improve the agency’s transfer process. The ideal candidate will have strong clinical skills, familiarity with agency programs, and the ability to navigate logistical challenges in client transitions.
Clinical Duties:
- Maintain a personal caseload, providing evidence-based therapy services.
- Develop and implement individualized treatment plans in collaboration with clients.
- Complete progress notes and other required documentation in a timely and compliant manner.
Transfer Coordination & Quality Assurance:
- Facilitate client transfers by assessing client needs, preferences, and therapist availability.
- Communicate with outgoing therapists to gather clinical insights and ensure continuity of care.
- Collaborate with supervisors and directors while maintaining final decision-making authority on therapist transfers.
- Match clients with available therapists based on clinical fit, scheduling needs, and program involvement.
- Assist with caseload transitions when clinicians leave the agency, ensuring minimal service disruption.
- Develop and refine policies to improve the efficiency and effectiveness of the transfer process.
- Master’s degree in Social Work, Counseling, Psychology, or a related field.
- Associate or Full License (LCSW, LMHC, LMFT, LPC, LSW, or equivalent).
- Familiarity or interest in learning about agency programs (e.g., DBT, IOP, school-based counseling) to support appropriate client matches.
- Strong organizational and problem-solving skills.
- Experience with electronic health record (EHR) systems preferred.
- Excellent communication skills and ability to collaborate across teams.
